Long Term Care
NPHS understands how severe cognitive impairment may require continual supervision and/or assistance in performing the activities of daily living (such as bathing, dressing, or getting up from a bed or chair).
Our long term care caregivers also assistance with day to day activities such as medication management, personal hygiene, light housekeeping or meal preparation.

Long term care can be provided in your home, an adult day care, assisted living facility or a nursing home.
